Malignant Melanoma is the most lethal of the skin cancers, and histological diagnosis is highly subjective and difficult, especially when biopsied by the shave technique.  The diagnosis is further complicated by the need to differentiate Malignant Melanoma from other melanocytic mimickers such as Spitz Nevus.

 

Acupath’s Melanoma and Spitz Nevus FISH Panels are used as an adjunct to conventional H&E and IHC studies, particularly in ambiguous cases, to identify genetic abnormalities that are confirmed to be specific to Malignant Melanoma and Spitz Nevus.  Diagnosis by the FISH technique can be performed on the same formalin-fixed, paraffin-embedded tissue used to make H&E slides.

Objective, Quantifiable Confirmation by FISH

AcuProbe Melanoma:  Acupath’s molecular pathologists have validated four specific genetic FISH (fluorescence in situ hybridization) probes that can detect relative gains and losses of genetic mutations which can differentiate Malignant Melanoma from benign melanocytic lesions.  These probes were developed based on previously performed studies.  The genes of interest are RREB1 (6p25 gain), MYB (6q23 loss), CCND1 (11q13 gain).  CEP6 is used as an internal control (Abbott Molecular).

 

AcuProbe Spitz:  An additional probe, HRAS (11p15) which is used in conjunction with CCND1 (used as an internal control), is unique to Acupath and has been observed in Spitz Nevi, especially Desmoplastic Spitz Nevi.

AcuProbe Melanoma Comprehensive Acupath dermatopathologists choose all IHC, special (billable) stains and FISH testing for diagnosis under consideration (Melanoma or Spitz Nevus) or as deemed medically necessary, unless otherwise indicated by ordering physician in the Test Request section on the requisition.

Acupath’s Melanoma and Spitz Nevus FISH probes are original designs and have been specifically developed to optimize the area of coverage allowing for an increased specificity and sensitivity.

How to submit a specimen 

The AcuProbe FISH test is useful for melanocytic neoplasms only, specifically Malignant Melanoma and Spitz Nevus.  Acupath will only perform the FISH test if the physician has indicated a clinical diagnosis of Melanoma or Spitz Nevus on the requisition.

 

PHYSICIANS

For new biopsies:

  • Submit tissue in formalin container as you would for routine histology. No additional specimen is required.

 

To request FISH testing on cases previously diagnosed by another lab:

  • Please contact the lab that diagnosed the case(s) and request shipment of the items below to Acupath.
  • Fax an Acupath requisition or letter (on your letterhead) to Acupath at 516-977-5396 or 516-977-6793 requesting the FISH test and include the patient(s) name(s) and the name of the lab who is sending us the items.
      • Paraffin block or 3-5 H&E slide with the area of interest circled • Pathology report with available patient history/clinical information
        unstained slides • All other representative IHC slides

 

LABORATORIES

For histology and FISH:

  • Submit tissue in formalin container as you would for routine histology. No additional specimen is required.

 

For FISH only: 

  • Please fill out an Acupath requisition and ship the items below to Acupath:
