Bladder tumor
 
HISTORY
A 60 year old male presents with a large bladder tumor.

Immunohistochemical stains were performed, showing positive staining of tumor cells with PCK (AE1/AE3) and S100. No staining is seen with CK 7, CK 20, K903, PSA, desmin, SMA, Alk-1, CD 30, Melan-A, Ki67 stains approximately 90% of cells. P53, CD 68 are non-contributory.
 
Figs. 1-5- Sections reveal highly atypical polygonal cells with marked nuclear atypia and pleomorphism infiltrating through the stroma. Many areas show spindle cell features, other areas show myxoid change, and others show a significant lymphoid infiltrate.
 
DIAGNOSIS
High grade urothelial carcinoma with predominantly sarcomatoid and focal lymphoepithelial features. Invasion into lamina propria is seen. Muscularis propria is focally present but does not show involvement by tumor in this biopsy
